Community Palliative Care

For some people it is very important culturally or psychologically, to be cared for at home in the early stage of requiring Palliative Care.

At MDHS we are able to provide Community Palliative Care for clients and patients with advanced, progressive and life limiting illnesses, both cancer and non-cancer related illnesses.  A referral from a General Practitioner is required to receive Community Palliative Care.

Our experienced Palliative Care nurses and District Nursing Service provide care, support and advice to clients, carers and their families.  Advice and support include assessment of Palliative Care needs, symptom management, psychological, social and emotional support, spiritual support (if requested), end of life and terminal care, access to aids and equipment and information on accessing other resources including dietitian, occupational therapy, social work and other Allied Health services as needed.
